LIABILITY RELEASE AND ASSUMPTION OF RISK

By registering for the Henderson Angels Pickleball Tournament 2026, I acknowledge and agree to the following:

1. ASSUMPTION OF RISK: I understand that participation in pickleball involves inherent risks including, but not limited to, physical injury, property damage, and other unforeseeable events. I voluntarily assume all such risks.

2. RELEASE OF LIABILITY: I release The Angel Charitable Corporation, its board members, officers, volunteers, sponsors, and Chicken N Pickle Henderson from any and all claims, damages, or liability arising from my participation.

3. MEDICAL AUTHORIZATION: In the event of injury, I authorize event organizers to seek emergency medical care on my behalf.

4. PHYSICAL FITNESS: I certify that I am physically fit to participate and have no medical conditions that would prevent safe participation.

5. PHOTO/MEDIA RELEASE: I grant The Angel Charitable Corporation permission to use photographs and video from the event for promotional purposes.

6. CODE OF CONDUCT: I agree to abide by tournament rules, treat all participants and staff with respect, and follow instructions from event organizers.

By completing registration, I acknowledge that I have read, understand, and agree to this waiver.
